Fix bug in Claims/Billing Portal – clear out claim tickets for previous patient when making patient payments

Indicate if practice location is inactive in Claims Search screens.  Allow users to search and create claims on Active and Inactive practice locations. 

Fixed display of patient name throughout Claims Portal so that data is consistently displayed as Last Name, First Name

Fixed bug in Claims/Billing Portal on insurance payments screen related to Payer IDs. 

Fixed issues that was causing Portal to error when attempting to create or edit provider schedule templates..
