• Improvements to CCDA document layout that is being produced for patient visit data

• Updated Patient Information Form report to include allergies, medications, symptoms, and referral reasons
• Fixed bug related to successful patient payments following declined payments (affected payment data being displayed on screen and on patient check out receipt report)
