Complete your Preregistration Paperwork

Your doctor's office may ask you to utilize the Complete.MD web portal to update or complete your preregistration paperwork.  To do so, follow the instructions below.  

First, navigate to  https://portal.complete.md/login and login with your email address and password.  

Click the Preregistration menu on the left hand menu.

Follow the prompts on the screen and use the buttons at the bottom of each section to walk through the Preregistration paperwork process.  Your answers will be saved and available for your provider's review at your next scheduled appointment.
